Why do two segments print when one note shows?

This one is regarding Case Number 2935 and Person Number 18661. When the worker goes to f2 for notes, it appears that there is only one set of notes. However, when she does to print her notes, two separate segments of notes print. If you do the f21 for list you can see those notes. What are we missing here and what exactly does f21 list mean. Can we get the notes to all appear when you choose f2 for notes?
