Home Enrollment Report


The report lists placement in each home for a specified date range. For each home selected, it shows Home Name, Client Number, Client Name, Age of client, Date Placement Began, and Date Placement Ended.

Selection: Home Status ‘O’=Open, ‘C’=Closed, ‘B’= Both, ‘T’=Terminated.
Includes only placements whose Date Began or Date Ended are within the date range.
By Worker ID.
By Home Number. (blank=All)
Provider defaults, usually to ‘CWS’.

Output Options: W=Wide Report
'W' means to wide report format (14x11 landscape, no page break)

Sequence: By Date Began within Home Number.
